Bon Losee Academy

1. Graduate from a school of Cosmetology/Barbering with at least 2000 hours.
2. Pass a written theory exam and law & rules exam (contact Experior, (801) 355-5009).
3. Pass a practical exam. (contact Experior, (801) 355-5009)
4. Make application and pay fees.

Requirements for Esthetics licenses:

1. Graduate from a school of Esthetics with at least 600 or 1200 hours (Basic or Master course, respectively.)
2. Pass a written theory exam and law & rules exam (contact Experior, (801) 355-5009).
3. Pass a practical exam. (Basic Esthetics only.) (contact Experior, (801) 355-5009).
4. Make application and pay fees.

EMPLOYMENT ASSISTANCE
The Academy will assist a student in obtaining employment upon graduation by maintaining a list of employers seeking qualified cosmetologists and/ or estheticians. Graduates may examine this list at any time. The Academy in no way guarantees employment.

ACCESS TO RECORDS AND RIGHT TO PRIVACY
Students have the right to inspect their school records by making a written request two days prior to the review with a statement defining the purpose. Student records will be reviewed with an authorized school representative. All student records shall be kept confidential and only released upon written authorization of the student or parent of dependent minor student, with the following exceptions:

1. Student records may be released to collection agencies for collection of funds owed the Academy.
2. Law enforcement agencies with proper legal procedures.
3. Parents of dependent minors may review a student's cumulative records with an authorized school representative present to interpret the records.
4. Student records may be released to applicable financial institutions. All reviews must be scheduled two days in advance.
5. Specified officials for audit or evaluation interest.
6. Other schools to which a student is transferring.
7. Appropriate parties in connection with financial aid to a student.
8. Organizations conducting certain studies for or on behalf of the school.
9. Accrediting organizations and government organizations.
10. Appropriate officials in case of health and safety emergencies.
